The total amount for DJ services is $_______.
A $ 100 deposit is due at the signing of this contract, payable to Will
Surrency.
The balance of $_______ will be due on 12/06/2008 (date) prior to the
engagement described herein.
Terms of Agreement

Unless otherwise agreed the
minimum deposit of $100.00 required to bind terms. Check should be
payable to Will Surrency.

Any deposit received is
non-refundable.

Parties entering into this
contract are permanently bound to fulfill the obligations by this
agreement unless one or more of the following conditions apply:
Both parties mutually agree to leave the agreement and its
responsibilities.
The event described herein is completely cancelled, and a minimum of 60
days notice is given.
In the event that Will Surrency becomes unable to provide his services
due to complications beyond his control, every attempt will be made to
schedule a reputable replacement. If replacement is not available, all
funds shall be refunded to client.

Any and all information or
details must be communicated before the 5 days preceding the
performance.

Will Surrency shall not be
held liable for any action arising from or in connection with any
individuals not directly affiliated with Will Surrency. Furthermore, the
party(s) contacting Will Surrency agrees to assume full responsibility
for any and all damages caused by themselves or their guests involving
any of the Will Surrency property.

The parties purchasing the
services of Jacksonville Party Time Entertainment/Will Surrency agree to
allow Jacksonville Party Time Entertainment/Will Surrency to use
pictures and video images taken at this event for promotional purposes
I understand the terms and conditions described herein, and will abide
by them.
